If you are a self-pay patient, please see our Price Sheet for our updated pricing.  If you have health insurance, be sure to review your specific insurance policy to know what portion of the visit cost you will be responsible for, and know that you can reach our billing company directly at 615-239-2018 if you have questions about a bill.

While we are able to accept a wide range of insurance plans, we recommend calling our office to verify your specific coverage prior to scheduling your appointment.

We understand that sometimes you need to cancel or reschedule your appointment due to unforeseen circumstances. If you are unable to keep your appointment, please call our office at (615)-895-6900 as soon as possible. 

If your appointment is cancelled less than 24 hours in advance or if you fail to appear within 15 minutes of your appointment time, a charge of $35 will be applied to your account.

Patients may be automatically discharged after three or more visits with no-show or late cancellation status.

We offer physicals year-round for school, sports, childcare, DOT, and annual visits.  Please call our office to schedule your next physical at (615)-895-6900.

A Department of Transportation (DOT) physical is a physical exam that is required of individuals who drive commercial vehicles. The purpose of a DOT physical is to ensure that people who are driving commercial motor vehicles are physically, mentally, and emotionally fit to carry out their duties safely. The Federal Motor Carrier Safety Administration (FMCSA) requires these of DOT employees for the safety of the public. We offer DOT physicals for $80.  Please call our office to schedule at (615)-895-6900.

We do offer aesthetic services for interested patients.  Botox and filler services are provided during scheduled aesthetic visits upon request, and medical-grade ear piercings are also available for both children and adults.  If you are interested in scheduling an aesthetics appointment, call our office at (615)-895-6900.
