If you are wanting to set up a new patient appointment, start by calling our office at (615)-895-6900. Our front office staff will create your chart, verify that your insurance is in-network, and schedule your first visit. We are accepting new patients of all ages - adults and children are welcome.
While online check-in can be helpful to keep your information up-to-date once you are established, we do require a hard copy of Patient Registration paperwork for all patients during the first visit of every year. If you are a new patient, we recommend bringing a copy of your completed paperwork to your first appointment to streamline the check-in process when you arrive.
If you are a self-pay patient, please be sure to review our Price Sheet prior to your appointment to financially plan for your visit. Know that the visit cost will be collected on the day of your appointment, and any services added during your visit will be billed accordingly. Our in-house services are all listed on the Price Sheet provided, and laboratory pricing through PathGroup is available during your visit upon request.
We recommend calling the Member Services number for your insurance - typically located on the back of your card - prior to your appointment to notify your insurance company that your PCP needs to be changed to whomever you are scheduled to see at your upcoming visit. This is especially important for Medicaid and Medicare patients. Some plans require your PCP to be changed before the appointment to ensure coverage, so this will save you stress in the future and paperwork in the office.
We completely understand that sometimes, you may need to bring additional people to your appointment. We do not have a set cap on guests at this time, but we do ask that patients are mindful of the limited lobby space and do not leave guests or children in the lobby while being seen for an appointment. This helps to ensure seating for patients who may be checking in during your appointment or may be waiting for a room with another provider.
On the day of your appointment, make sure to come prepared with your ID, insurance cards, and any medications you are currently taking. We will need copies of your ID and insurance cards for your chart, and our nurses will review your medications to ensure an up-to-date list on your medical chart and note any medications that need refills.
